Mental Health Community Integration Worker
The primary role of the Community Integration Worker is to assess, coordinate, link, refer, and monitor services for a consumer that promote the consumer’s independence. The Community Integration Worker works with the consumer, natural supports, and all area service providers involved, to achieve measurable success in the goal areas that have been established/identified, by the consumer that is reflected on there individual service plan (ISP).
Being dependable (showing up for work when scheduled) and punctual (being on time for your shifts) is extremely important because the consumers count on staff being there and this is important in building co-worker relationships.
