Mental Retardation Community Case Manager
The primary role of a community case manager is to
assist in identifying and implementing support strategies that reflect
the consumer’s personal vision.
The case manager works with the consumer and other individuals
that the consumer identifies such as family, friends, and service
providers in developing a person centered plan and then assists with
follow through on goals and objectives.
The case manager will monitor services and work with the consumer
to assure his/her satisfaction with the process and outcomes of support,
services and available resources.
As an employee of ESM it’s important to
be flexible. ESM serves
individuals around the clock.
Consumers can have a variety of hours from Day shift, evening
shifts, and overnight shifts.
Weekends and Holidays are also important as ESM operates twenty
four hours a day seven days a week.
Being dependable (showing up for work
when scheduled) and punctual (being on time for your shifts) is
extremely important because the consumers count on staff being there and
this is important in building co-worker relationships.
