Community Case Management ~ CCM

Community Case Management (CCM) is a community based program providing services to adults with Intellectual Disabilities. CCM staff assist in identifying medical, social, educational, and other needs of consumer. CCM staff will address and help to facilitate access to these needed services. The case manager will also work with the individual and family to identify any wishes or desires and assist with achieving these goals. Case management involves plan of care development, coordination/advocacy, goal development, monitoring, and evaluation.
Community Integration ~ CI
(MH)
Community Integration Services (CIS) is a community based program designed to assist adults with Mental Illness identify the services and support they need to live successfully in their community. A Community Integration Worker (CIW) will be assigned to help the consumer identify their needs and desires when a referral is received. The CIW will help the consumer with the planning and linking of needed services. The CIW will also monitor the consumer’s services on a regular basis to ensure the consumer’s needs are being met.
Targeted Case Management Services ~TCM
(Children)
ESM’s Children’s Targeted Case Management program is for children and adolescents (Ages 0-20) with Mental Health, Intellectual Disabilities and/or Autism diagnoses. We are here to support you and your child through assessing and identifying your child’s strengths and needs, coordinating and developing a service plan to address those needs, and monitoring the plan to ensure effectiveness and outcomes. We can help address needs in many areas including but not limited to: psychological, vocational, educational, living skills, behavioral, medical, transition, and legal. Our goal is to enhance children’s functioning within the home and community.
